You may apply for admission to Lake Country School in either of the following ways:

1. To apply online, click here. After you have created an applicant, click on "Complete Form" to continue with the application process. You will be asked to pay an application fee using a credit card. If you are reapplying or intend to apply for tuition assistance please call Lake Country School for a fee waiver code to proceed with the online application process.

2. To download a PDF application form, click here. This form can be printed and filled out by hand, or filled out and saved on your computer. Mac/Apple users please note: the form should be completed using Adobe Reader; please do not use Apple's Preview program.
